Proof of Residency Requirement

Prior to a student attending one of the public schools in the Polk County School District, a parent/guardian must provide proof of residency to the Polk County School Board.

What to do: Bring any TWO of the following documents to the school that your children are zoned to attend (If you do not know which school your children should attend, you may contact the Pupil Accounting and Records Office).
bulletApartment or home lease agreement, mortgage document, or property tax record
bulletCurrent utility bill (electric, gas, phone, cable, water)
bulletVoter registration document
bulletProof of government benefits (disability, Medicare, food stamps, HRS correspondence)

If you are not able to provide any TWO of the documents listed above, you may use an Affidavit of Residency. The affidavit must be notarized by a notary public of the State of Florida.
